This form is to cancel your membership. Submitting it is your formal notice of cancellation. It is not an enquiry, and your cancellation will be processed as submitted. You’ll receive confirmation of your final billing and end date by email within 7 days.

• You must have completed your minimum contract term. We check this in our system. If it isn’t complete yet, we’ll set your cancellation for the end of your minimum term.

• Cancellations require 30 days’ notice (4 payments), which are charged as normal.

• If you re-join in future, sign-up fees apply and you’ll need to choose from the memberships available in the app at that time. Some memberships aren’t offered year-round (for example, over summer).
